Homeless Prevention & Rapid Re-Housing

As part of the Fairfax-Falls Church 10 Year Plan to Prevent and End Homelessness, a coordinated approach, called HOST (Housing Opportunities Support Team), works to identify persons at risk of homelessness and to intervene to prevent homelessness or divert persons from shelters. Federal stimulus funds (HPRP) are now available to provide rent subsidy and other housing-related financial assistance for up to 18 months, on a 3-month renewable basis. Fairfax County has contracted with New Hope Housing for a senior community case manager to coordinate HPRP prevention efforts in south county.
